Phoenix Certified Hospice
is a special kind of care designed to provide sensitive and supportive
care for people with life limiting illnesses. Phoenix Certified Hospice
care seeks
to enable the patient to carry on an alert, pain-free life and to
manage other symptoms so that their last days may be spent with
dignity and quality at home.
Phoenix Certified Hospice
should be considered when a patient and the physician determine
that curative treatment is no longer effective or desired, and life
expectancy can be measured in weeks or months rather than years.
Referrals can be made by physicians or social service agencies,
as well as direct requests by the patient, family or primary caregiver.
The patient must have a local physician who will accept responsibility
of the overall care. It is preferred that an individual has a willing
and able caregiving person (or persons) at home.
Phoenix Certified Hospice
offers comfort care, rather than curative treatment. Under the direction
of a physician, Phoenix Certified Hospice uses sophisticated methods of pain and symptom
control that enable the patient to live as fully and comfortably
as possible. Phoenix Certified Hospice treats the person, not the disease. The interdisciplinary
hospice team is made up of professionals who address the medical,
emotional, psychological, and spiritual needs of the patient and
family. Phoenix Certified Hospice service is provided by a team of trained professionals;
physicians, nurses, counselors, therapists and aides of the Phoenix Certified Hospice
interdisciplinary team.
